NEW PATIENTS

Thank you for visiting the Prosthodontic Associates website! Our goal is for your first visit to be one of comfort and acknowledgement of your personal dental needs.

We will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that is tailored to your individual needs and expectations. We will complete a thorough comprehensive dental evaluation.

Why? Because the foundation of a long term relationship is established during your first visit at our practice..

First Visit

Please arrive 15 minutes early to your first appointment in order to check-in with the building’s welcome center and fill out paperwork. Your initial appointment will consist of a consultation, explaining the diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment.

Please assist us by providing the following information at the time of your consultation:

• Any available x-rays (Please have your previous dentists email x-rays in .dexis or .jpeg format to [email protected] prior to visit)
• Fill out the required admission forms 
• A list of medications you are currently taking
• If you have out-of-network coverage, please bring insurance information

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

X-Rays

In addition to any previous x-rays, a new complete set may be necessary depending on your dental needs. X-rays are another instrument in the repetoire that assist in accurately diagnosing and treatment planning.
